If you have already seen Dr Myers you do not need to fill in the Online Questionnaire again unless your details have changed or you have a new problem.  ( See link Below)

You need to have a valid Referral.ie provided in the last 12 months.

Hot Docs is a new direct online Booking platform. The appointment will be confirmed by my Secretary.

Alternatively you can telephone Denise or Georgia on 02 9650 4382.

 

Please note:

1. A G.P. referral lasts 12 months

2. A referral from a Specialist only lasts 3 months.

 

If you have a NEW condition or a different problem you need a new referral.
